1. Acceptance of Terms

By booking an appointment, accessing our website, or utilizing any services provided by My Spazio Relax, you acknowledge that you have read, understood, and agree to be bound by these Terms and Conditions in their entirety. If you do not agree to these terms, please do not book or use our services.

These Terms and Conditions apply to all clients, whether services are rendered at our location or through outcall/mobile appointments.

1.1 SMS/Text Message Terms

By providing your phone number and submitting a form on our website, booking an appointment, or contacting My Spazio Relax, you agree to receive SMS/text messages from My Spazio Relax. These messages may include appointment confirmations, appointment reminders, intake form reminders, service updates, and occasional promotional messages.

Example:
You have successfully opted in to receive notifications and promotional SMS from My Spazio Relax. Please reply STOP if you wish to opt out.

You can cancel the SMS service at any time. Simply text STOP to unsubscribe. After you send STOP, we will confirm your unsubscribe status by SMS. After this confirmation, you will no longer receive SMS messages from us. To rejoin, you may sign up again as you did initially, and we will resume sending SMS messages to you.

If you experience issues with the messaging program, reply HELP for assistance, or contact us directly at [email protected] or call (478) 449-2007.

Carriers are not liable for delayed or undelivered messages.

Message and data rates may apply for messages sent to you from us and to us from you. Message frequency may vary depending on your appointment activity, service requests, and communication preferences. For questions about your text or data plan, please contact your wireless provider.

2. Services Offered

My Spazio Relax is owned and operated by Yolimar Malave, a licensed massage therapy professional in the State of Georgia. We offer the following professional wellness services:

•  Deep Healing Ritual Massage (Deep Tissue)

•  Swedish Restore & Relax Ritual

•  Harmony Hands & Feet Treatment

All services are provided by appointment only. My Spazio Relax reserves the right to add, modify, or discontinue any service at any time without prior notice.

3. Appointments & Booking

All services are by appointment only. Appointments may be booked through our website at www.myspaziorelax.com or by contacting us directly.

3.1 Appointment Confirmation

Your appointment is not confirmed until you receive a written confirmation via email or SMS. My Spazio Relax reserves the right to decline or reschedule any appointment at its discretion.

3.2 Intake Forms

All clients are required to complete a health intake form prior to their first appointment. This information is used solely to provide safe, effective, and personalized care. Clients are responsible for providing accurate, complete, and up-to-date health information. Failure to disclose relevant health conditions may affect the safety or appropriateness of services rendered.

3.3 Arrival

For in-studio appointments, clients are encouraged to arrive 5–10 minutes early. Late arrivals may result in a shortened session at the full scheduled price, at the discretion of the therapist.

4. Cancellation, Rescheduling & No-Show Policy

My Spazio Relax understands that life is unpredictable. We ask that you respect our therapist’s time and schedule by adhering to the following policy:

4.1 Cancellation & Rescheduling

We require at least 24 hours’ notice for any cancellations or rescheduling requests. Cancellations or rescheduling requests made with less than 24 hours’ notice will be charged 50% of the scheduled service price.

4.2 No-Shows

A no-show is defined as a failure to attend a scheduled appointment without any prior notice. No-shows will be charged 50% of the scheduled service price. Clients with repeated no-shows may be required to prepay for future appointments or may be refused future bookings.

4.3 Deposits

Any deposit made at the time of booking will be held as a credit toward services for up to 30 days from the original appointment date. Deposits are non-refundable but may be applied to a rescheduled appointment if proper notice is given.

4.4 Cancellations by My Spazio Relax

In the event that My Spazio Relax must cancel or reschedule your appointment, we will notify you as soon as possible and offer a full refund of any deposit or prepayment, or reschedule at your convenience.

5. Payment Terms

5.1 Accepted Payment Methods

My Spazio Relax accepts payment via credit and debit cards through Stripe, a secure third-party payment processor. By providing your payment information, you authorize My Spazio Relax to charge the applicable fees for services rendered.

5.2 Pricing

All service pricing is listed on our website and is subject to change without prior notice. The price applicable to your appointment will be confirmed at the time of booking.

5.3 Payment Due

Payment is due at the time of service unless prior arrangements have been made. My Spazio Relax does not accept insurance. We do not participate in any government or private health insurance billing programs.

5.4 Refunds

All sales are final. Refunds will not be issued for completed services. If you are dissatisfied with your experience, please contact us directly so we can address your concerns.

6. Outcall / Mobile Services

My Spazio Relax offers mobile outcall services, bringing our luxury wellness experience directly to your location. The following additional terms apply to outcall appointments:

Travel is complimentary within 4 miles of 149 Sourwood Ln, Warner Robins, GA 31093. For locations beyond this complimentary travel area, an additional travel fee of $1 per mile will apply.

Clients requesting outcall services are responsible for providing a clean, safe, and accessible space adequate for massage table setup. My Spazio Relax reserves the right to refuse or terminate an outcall appointment if the environment is deemed unsafe, unsanitary, or otherwise unsuitable, at which point the cancellation policy will apply. 

7. Right to Refuse Service

My Spazio Relax reserves the right to refuse or discontinue service to any client at any time for reasons including, but not limited to:

•        Safety, health, or medical contraindications

•        Inappropriate, threatening, or disrespectful behavior toward the therapist

•        Violation of these Terms and Conditions

•        Any conduct that compromises the professionalism or safety of the session

If a session is terminated early due to client behavior, the full session fee will be charged. My Spazio Relax maintains a strict zero-tolerance policy for sexual harassment or inappropriate requests of any kind. Such incidents will result in immediate termination of the session, full charges, and may be reported to law enforcement.

8. Health Disclosures & Medical Disclaimer

Massage therapy and wellness services provided by My Spazio Relax are not a substitute for professional medical advice, diagnosis, or treatment. Our services are intended for relaxation, wellness, and general well-being.

Clients are responsible for disclosing all relevant health conditions, medications, allergies, recent surgeries, injuries, or other medical information that may affect their treatment. My Spazio Relax will not be held liable for adverse effects resulting from incomplete or inaccurate health disclosures.

8.1 Prenatal Services

From the first trimester through the second trimester, medical authorization from the client’s doctor is required in order to receive a full body massage, however, if the client does not wish to receive a full body massage reflexology for the hands and feet may be performed instead.

8.2 Contraindications

If you have any of the following conditions, please consult your physician before booking a service: open wounds or skin infections, fever or contagious illness, blood clots or deep vein thrombosis, recent surgery, or any condition your physician has advised against massage therapy. My Spazio Relax reserves the right to reschedule or decline service if a contraindication is identified.

9. Waiver & Release of Liability

9.1 Purpose of Treatment

I understand that the rituals and treatments provided at My Spazio Relax are intended for the primary purpose of improving lymphatic flow and promoting deep relaxation. If I experience any pain or discomfort during the session, I agree to immediately inform the specialist so that the pressure or techniques may be adjusted to my level of comfort.

9.2 Medical Disclaimer

I acknowledge that massage and bodywork should not be construed as a substitute for medical examination, diagnosis, or treatment. I understand that My Spazio Relax specialists are not qualified to perform spinal or skeletal adjustments, nor are they authorized to diagnose, prescribe, or treat any physical or mental illness. Nothing said or done during the session should be construed as medical advice.

9.3 Contraindications & Health History

I affirm that I have disclosed all known medical conditions and answered all health questions honestly. I agree to keep my specialist updated on any changes to my medical profile. I understand there shall be no liability on the part of My Spazio Relax should I fail to provide accurate health information. I acknowledge that I should consult a physician before booking if I have kidney or thyroid problems, bronchial asthma, active chemotherapy, prescribed cardiovascular medications, or are in the first trimester of pregnancy.

9.4 Understanding Side Effects

I understand that side effects associated with lymphatic and massage therapy may include, but are not limited to: fatigue or tiredness due to toxin release; tenderness around blocked lymph nodes; sensations of being chilled; superficial bruising; increased thirst or urination; and brief dizziness when transitioning off the table.

9.5 Professional Standards

All sessions at My Spazio Relax are strictly therapeutic and non-sexual in nature. Any inappropriate remarks or behavior will result in immediate termination of the session without a refund and may be reported to law enforcement as outlined in Section 7 of these Terms.

9.6 Acknowledgment & Electronic Signature

By booking a ritual and typing my name and email during the intake process, I acknowledge and agree to this Waiver & Release of Liability. I hereby waive and release My Spazio Relax and its treatment providers from any and all liability—past, present, and future—relating to massage therapy and bodywork. I agree that my electronic signature (typed name) carries the same legal force and effect as a handwritten signature.

10. Age Requirements

My Spazio Relax services are available to clients who are 18 years of age or older. Minors under the age of 18 may receive services only when accompanied by a parent or legal guardian. By booking or receiving services for a minor, the parent or legal guardian confirms that they have the legal authority to provide consent on the minor’s behalf.

11. Privacy & HIPAA

My Spazio Relax is committed to protecting the privacy of our clients’ personal and health information. All personal and health information collected through our website, intake forms, or booking platform is handled in accordance with applicable privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA).

Personal information is never sold or shared with third parties for marketing purposes.

12. Intellectual Property

All content on the My Spazio Relax website, including text, images, logos, graphics, and branding, is the property of My Spazio Relax and is protected by applicable copyright and intellectual property laws. Unauthorized use, reproduction, or distribution of any content is strictly prohibited.

13. Limitation of Liability

To the fullest extent permitted by Georgia law, My Spazio Relax shall not be liable for any indirect, incidental, special, or consequential damages arising from the use of our services or website. Our total liability for any claim arising under these Terms shall not exceed the amount paid by the client for the specific service giving rise to the claim.

My Spazio Relax shall not be responsible for any loss, theft, or damage to personal property brought to in-studio or outcall appointments.

14. Governing Law

These Terms and Conditions shall be governed by and construed in accordance with the laws of the State of Georgia, without regard to its conflict of law provisions. Any disputes arising under these Terms shall be subject to the exclusive jurisdiction of the courts located in Houston County, Georgia.

15. Changes to These Terms

My Spazio Relax reserves the right to update or modify these Terms and Conditions at any time without prior notice. The most current version will always be available on our website at www.myspaziorelax.com. Continued use of our services following any changes constitutes your acceptance of the revised terms.
